Chicago Read Mental Health Center is seeking a vendor to provide asphalt & concrete repair services across its campus located at 4200 N. Oak Park Ave., Chicago, Il 60634. This procurement is for Small Business Set Aside Program Vendors Only. Repair and repave campus roads to remove existing potholes-Provide Labor and Equipment to Repair Facility Road Potholes. Purchase and install pothole patching for the facility. Work to include removal and disposal of loose material, apply tack coat, install blacktop patch, and compact with roller.
